To make it as easier for our parents/caregivers to access the forms and documentation, below is a list of our most common forms.  If you require any further forms or information, please contact the office.

Your time is valuable, so we encourage parents/caregivers to fill out our forms in advance.

Download and complete your forms ahead of time and email, to the school office, or drop them into the office next time you visit the school.

Medical and Health Care Information

Students who need health care support at school must have a management and emergency response plan.

It is a Department of Education requirement that any medication administered at school, must first be authorised in writing by the parent/caregiver.

Health Care Plans must be signed and dated by a parent/caregiver and provided to the school with the medication.

Requests for staff to administer medication must be made at the office.

Parent/caregivers are asked not to make individual requests to the teacher.

Staff will not administer any medication (short or long term) without written authorisation.

Department of Education policy states:

Parents must provide medication that is:

  • clearly labelled with the student's name.
  • within the expiration date.
  • in original packaging.

Enrolment Forms

Please visit our Enrolments page  for specific information about our enrolment process. 

  • The enrolment is a two-step process.
  • Step 1.   Application for Enrolment. Complete the Application for Enrolment, and email and attached documents listed below.
  • Step 2. Student Enrolment Form. Once the school has approved your Application for Enrolment, you will be contacted to complete the Student Enrolment form, student consent forms, and permissions paperwork.
