Once you've completed the necessary forms, you can either email them back to us or drop them off at the front office. If you need additional forms or have any questions, please don't hesitate to contact us during school hours.

Change of Student Contact Information - Update contact information for parent/caregivers, and additional emergency contacts.
In-Term Vacation Request - For families requesting a leave of absence during term.
IMMS Form - Loan Agreement Form.
Parent-initiated service provider for students - This form request for external service providers such as therapists for children during school hours.
Student Exit Form - For families relocating to another area school, interstate or overseas.
Statutory Declaration - A statutory declaration is a written statement declared to be true in the presence of an authorised witness.
Student Smartrider Application Form - Complete the application , have it authorised by your school administration, and mail it to the address shown on the form.
Volunteer Declaration Form - This form must be completed by parents / grandparents /or visitors volunteering in child-related work at a public school or site.

Students who need health care support at school must have a management and emergency response plan.

It is a Department of Education requirement that any medication administered at school, must first be authorised in writing by the parent/caregiver.

Health Care Plans must be signed and dated by a parent/caregiver and provided to the school with the medication.

Requests for staff to administer medication must be made at the office.

Parent/caregivers are asked not to make individual requests to the teacher.

Staff will not administer any medication (short or long term) without written authorisation.

Department of Education policy states:

Parents must provide medication that is:

clearly labelled with the student's name.
within the expiration date.
in original packaging.

Student Health Care Summary - (Form 1) Provided to parents/caregivers at enrolment, reviewed annually, or if student health care needs change during the year.
Generic Health Care Plan - (Form 2) For all other health care needs. e.g., ADD/ADHD, ritalin medication.
Administration of Medication - (Form 3) To administer medication to a student for a short-term period. e.g. antibiotics, paracetamol.
Severe Allergy/Anaphylaxis - (Form 4) To be completed with Anaphylaxis Action Plan, which must be signed local GP.
Anaphylaxis Action Plan - To be completed and returned with (Form 4).
Mild and Moderate Allergies Plan - (Form 5) For students who have mild allergies to bees, nuts, or medication.
Diabetes Action and Management Plan - For further information, please see Diabetes best practice guidelines.
Seizure Management Plan - (Form 7) To develop a plan for students who have epilepsy and/or seizures.
Asthma Management Plan - (Form 8) For students with Asthma. This plan must be signed by parents/caregivers and by a GP.
Activity of Daily Living - (Form 9) To develop a plan for students who need support with activities of daily living.
Emergency response plan ESCU.-1 - (Form 10) To develop a plan for students with special needs.

The enrolment is a two-step process.
Step 1. Application for Enrolment. Complete the Application for Enrolment, and email and attached documents listed below.
Step 2. Student Enrolment Form. Once the school has approved your Application for Enrolment, you will be contacted to complete the Student Enrolment form, student consent forms, and permissions paperwork.
